*You must complete and include BOTH forms on the next page along with your payment.
*Please do not forget to sign and date the bottom of the second form.
*All dogs must be on a leash (carriers/kennels are recommended). All cats must be in a kennel or carrier at all times.
*No pets under 4 months of age are allowed to enter due to lack of rabies
vaccinations.
To enter a dog or cat in this show, you will need to print out and complete the two page form which is linked to on this page.
It is saved in JPEG format (form1.jpg and form2.jpg). To print this you have two options. After clicking on the link below you can either:
1)Right click with your mouse above the form and move your mouse downward until you see the two forms are both highlighted (i.e. the writing will look gray and fuzzy). Then right click on the word "file" up at the top of your browser and click on print in the drop down menu. When you do this a print window will pop up. In it click by the word "selection", then click ok.
2)You can download the file to your computer by left clicking on it and selecting "save picture as" in the box that pops up. Then click on save (remember to look in the top box to see where the picture is being saved so you don't lose it). You will have to save both the top form and the bottom form seperately. Then you can print them out with the program you use to view JPEG images.
If neither of these options work for you, or the instructions aren't clear (I'm sorry if they're not), please email AHSSP@hotmail.com and we will send you a copy of the form.
PLEASE SEND IN FORMS ASAP! FOR THE $10 ENTRY FEE FORMS MUST BE IN OUR HANDS BY 5:00PM ON FEB. 21ST. IF THE FORMS ARE BROUGHT ON THE DAY OF THE EVENT, ENTRY FEE INCREASES TO $15!
